Position Summary


Title: Senior Services Coordinator

Work Location: 415 ½ W Ashland Avenue, Louisville, KY 40214 

The Senior Services Coordinator is a full-time position and reports directly to the Assistant Director. We are seeking an enthusiastic and compassionate individual who believes in our mission and embodies our values to join our dynamic and innovative team.

Responsibilities and Duties

  • Contact individuals who request/receive Meal on Wheels to determine contact information, and conduct initial home visits. 

  • Maintain the Senior Commodity Program and SLCM homebound grocery delivery program in coordination with Dare to Care including annual audits and recordkeeping.

  • Provide at a minimum of annual in-person reviews for all senior programs and conduct ongoing client assessments to identify challenges, goals and necessary interventions.

  • Maintain required participant records as needed by KIPDA and Nutrition Program, including regularly updating neighbor meal units in the Mon Ami program, as well as all tracking spreadsheets required for each program. 

  • Work with our partners to ensure the distribution of the meals weekly.

  • Develop and redesign efficient delivery routes for meal programs.

  • Deliver meals in the absence of volunteers.

  • Train and supervise agency volunteers for the senior meal and social inclusion programs.

  • Coordinate team members in social inclusion activities, primarily phone calls, to connect with the program participants.

  • Other related duties as assigned

The successful candidate will have:

  • A BA/BS in the area of social work (or related field) (REQUIRED)

  • Regular, reliable transportation (REQUIRED)

  • The ability to lift 40lbs (REQUIRED)

  • Demonstrated recent experience working in direct client services

  • Excellent organizational skills

  • Excellent interpersonal, verbal and written communication skills

  • Enthusiasm and commitment to the vision and mission of the agency

  • Communication Skills: Strong verbal and written communication skills to effectively interact with team members, Neighbors, faith communities, healthcare providers, and social service agencies.

  • Problem-Solving: Proven ability to assess clients' needs, identify barriers to care, and develop appropriate solutions.

  • Organizational Skills: Ability to manage diverse and, sometimes, competing priorities. Effective project and talent management capabilities. 

  • Technical Capacity: Working knowledge of Google Office platform, project management and other database functions. 

  • Social Services: Knowledge of available community resources and social services that can support seniors with housing, transportation, food, and other social determinants of health.

  • Cultural Competency: Culturally sensitive and capable of working with diverse populations, understanding and respecting differences.

Compensation: 

This is a 40 hour/week full-time position: $40,000 per year.  Benefits include 100% self-only medical insurance plus dental/vision, life insurance/short-term disability, travel reimbursement, 3% retirement match (403b), competitive paid vacation and sick leave, paid holidays and professional development opportunities.
